Meropenem as a Treatment for Nocardia Scleritis

Purpose: To present a case of meropenem treated Nocardia scleritis in a penicillin and sulfa allergic patient.

Methods: A 66 year old male with history of scleral buckle for retinal detachment presented with necrotizing scleritis, referred after treatment for autoimmune scleritis. Removal of scleral buckle and cultures revealed Nocardia infection. Patient was allergic to penicillin and sulfa medications. Hospitalization and consultation with infectious disease resulted in treatment protocol of intravenous (IV) meropenem, oral doxycycline, and topical fluoroquinolone, vancomycin, and amikacin. Extended treatment resulted in resolution of infection.

Results: Nocardia infection was controlled and eliminated in a penicillin and sulfa allergic patient with IV meropenem in conjunction with topical therapies.

Conclusion: IV meropenem is an effective therapy for Nocardia scleritis in conjunction with topical adjunctive medications.
